IgA腎症における扁桃誘発試験と扁摘効果

Tonsillar Provocation Test and Effect of Tonsillectomy in Patients with IgA Nephropathy

　はじめに

　IgA腎症は，1968年Berger^1）が報告したのが最初で，扁桃病巣感染症の代表的疾患の1つとされており，本邦では1983年の杉山ら^2），相馬ら^3），山辺ら^4）の報告以来，口蓋扁桃摘出術（以下，扁摘）の有効性が報告されてきた。

　扁桃病巣感染症の補助診断法の1つとして，扁桃誘発試験（以下，誘発試験）が日常臨床において広く用いられている。しかし刺激方法，刺激時間，パラメータの選択，陽性判定基準，陽性判定時間，誘発試験成績と扁摘効果が一致しないなど，様々な問題点が指摘されてきた^5,6）。これらの点に対して「扁桃病巣感染症診断基準の標準化に関する委員会」は，標準化作業のための基準を呈示した^7）。また，超短波誘発試験法のうち直接導子を扁桃に当てて照射する直接誘発法（以下，直接法）と，導子を下顎角部などに当て皮膚・筋肉・骨などを通して扁桃に照射する間接誘発法（以下，間接法）を比較した場合，信頼性が高いのは直接法であると報告されてきた^8,9）。

　今回われわれは，「扁桃病巣感染症診断基準の標準化に関する委員会」のプロトコールに従って誘発試験（直接法または間接法）を行った後，扁摘を施行し6か月以上経過観察のできたIgA腎症例を対象に，各々の誘発試験陽性率および診断的中率を出し，プロトコールに示された方法による直接法と間接法のIgA腎症に対する補助診断法としての有用性について，比較検討を行ったので報告する。

We studied the relationship between the results of tonsillar provocation tests and the effects of tonsillectomy in 47 patients, including 21 males and 26 females, with IgA nephropathy. All patients were followed-up for more than 6 months after tonsillectomy.

The postoperative remission ratio was 53.2% (25/ 47) for proteinuria. The positive ratio was 86.7% in the tonsillar provocation tests by the direct irradia-tion of ultra-micro waves, and 81.8% by the indirect irradiation method. The correct ratios of diagnosis were 53.3% by the direct method, and 45.5% by the indirect method respectively.
