インターフェロン投与中に感音難聴を生じた3症例

Acute Sensorineural Hearing Loss during Interferon Therapy:Report of Three Cases

　はじめに

　近年慢性活動性肝炎に対し，インターフェロン療法が積極的に行われるようになってきている。それに伴い精神神経症状^1），眼症状^2），甲状腺機能異常^3）など種々の副作用が臨床の場において報告されてきている。耳鼻科領域における副作用としては難聴，平衡障害，耳鳴などの報告^4〜8）があり，何らかの内耳障害が示唆されているが，報告例も少なく，その詳しい機序に関しては不明の点が多い。現在までのインターフェロン（以下IFNと略）投与中に生じた感音性難聴例では，ほとんどの症例が投与中止後に聴力の回復をみている。今回われわれの経験したIFN投与中に骨導閾値上昇をきたした3症例では，これまでの報告と異なりIFN中止後も聴力改善がほとんど認められなかった。本稿では，これらの3症例を報告するとともに，若干の文献的考察を加え，その作用機序について検討を加えた。

Three patients had been treated by interferon injection for chronic active hepatitis. During the therapy, they complained of a hearing disturbance and were diagnosed as having acute sensorineural hearing loss. We immediately abandoned the injec-tion of interferon in 2 patients, and the other kept on receiving the injection. Steroids were not given to any patients. While one patient showed a slight improvement of hearing, the remaining 2 showed no improvement. From the audiological findings and clinical course of the three patients and of previous case reports, it was suggested that the hearing loss might be caused by autoimmune disorder in the inner ear.
