唾液腺腫瘍に対する穿刺吸引細胞診の有用性

Availability of Fine Needle Aspiration Biopsy Cytology for Salivary Gland Tumor

　はじめに

　唾液腺腫瘍は，良性，悪性を問わず多彩な組織像を呈する。また腫瘍以外にも炎症性腫瘤や，腫瘍類似疾患などの病変も多い。

　耳下腺に好発する腫瘍として，多形腺腫およびワルチン腫瘍があげられるが^1），臨床的に良性腫瘍を疑って手術した後，摘出標本で組織学的に悪性と診断される症例も少なくない。したがって，これらの腫瘍の性質が術前に分かることは，術式の決定に際して非常に有用である。しかし，唾液腺腫瘍に対する切除生検および大口径針による針生検は，穿刺経路への腫瘍細胞の播種の危険を伴い一般に禁忌とされている^2）。一方，穿刺吸引細胞診（fine needle aspiration biopsy：FNA）は，操作が簡単，安全であり，その結果が迅速に得られるため，近年，唾液腺腫瘍に対して施行される頻度が高まりつつある^3）。

　今回われわれは，唾液腺腫瘍の良性，悪性の鑑別ならびに組織型推定のおけるFNAの有用性について検討してみたので，若干の文献的考察を加えて報告する。

Over the past 4 years in Shinshu University Hospital and 3 years in Matsumoto Hospital and Syo-uwa-Inan Hospital and Iida Hospital, 59 cases of fine needle aspiration biopsy (FNA) of salivary gland tumors were investigated, and the result showed 12 cases of malignant tumor, 2 false positive and 6 histologically misdiagnosed cases.

From the valuable findings and minimal pain FNA should be more frequently applied in the oto-rhino-laryngologic field.
