はじめに
前下小脳動脈(AICA)に障害が起こると多彩な耳科的症状を呈し,これらはしばしばメニエール病や突発性難聴と鑑別が困難なことがある。しかし,十分な神経耳科学的検査および画像検査を行うことによって病巣診断ができうると考えられる。今回われわれは,急性感音難聴と回転性のめまいで発症した前下小脳動脈領域の脳幹梗塞の1例を経験したので報告する。
We encountered a patient with acute sensorineur-al hearing impairment and disequilibrium which were suggested an infarct in the territory of the anterior inferior cerebellar artery (AICA).
The patient initially developed Ménière's disease-like symptoms followed by facial nerve palsy, mild hypesthesia on the face and dysarthria. MRI showed a lesion suggesting AICA territory infarc-tion. Since the AICA supplies the vestibular nuclei, vestibulocochlear nerve and inner ear, any disturb-ances of this vessel can cause various otological symptoms. Therefore, thorough neurological and neuro-otological examination as well as imaging studies, particularly MRI, are important for distin-guishing brainstem infarction from inner ear dis-eases, such as Ménière's disease or sudden deafness.
