　はじめに

　術後性上顎嚢胞は顔面の疼痛をきたす耳鼻科的疾患としてよく知られている。症候性三叉神経痛のうち一般的に頬部から上顎歯敵部にかけての三叉神経第Ⅱ枝領域の放散痛を訴え，さらに頬部腫脹や複視，眼球突出，眼窩深部痛などの眼症状があり，副鼻腔手術の既往があれば比較的診断は容易な疾患である。われわれは，嚢胞が側頭下窩に進展した術後性上顎嚢胞で，三叉神経第Ⅲ枝領域痛を初発臨床症状としたため，あたかも特発性三叉神経痛のようであった症例を経験したので報告する。

38-year-old female complained of a left facial pain. A intractable pain was suddenly occurred and Confined to the lower law area and the temporoman-dibular joint region. A true trigeminal neuralgia of the mandibular nerve was suspected. Typical symp-toms due to maxillary cyst, such as a swelling of the cheek or upper toothache, were not complained in this ease. CT scan revealed a destructive finding of the posterior wall of the maxillary sinus. The final diagnosis was a postoperative maxillar-y cyst expanding extramurally into the left in-fratemporal fossa.
