　はじめに

　喉頭の腫瘍がわれわれ頭頸部外科医にとって割合よく遭遇する疾患である一方で，気管原発の腫瘍は喉頭に隣接する領域でありながら，経験する機会に恵まれない疾患である。しかし，気管でもその上方の頸部気管は隣接臓器に甲状腺や下咽頭・頸部食道を有し，手術の際に目にする部位であり，操作する機会は多いといえる。気管原発腫瘍は喉頭腫瘍と同じ上気道疾患であることから，われわれ頭頸部外科医が扱うべき疾患であり，たとえその頻度が少ないとはいえ知っておかねばならない疾患であるといえる。

　気管腫瘍，なかでも悪性腫瘍は，本邦では腺様嚢胞癌が多く，扁平上皮癌は少ないとされている^1,2）。最近われわれは気管原発と考えられた扁平上皮癌を治療する経験を得たので，若干の文献的考察を加え報告する。

Tumor of the trachea is rare disease in the head and neck region, and it can produce cough, dyspnea and hemoptysis. A 60-year-old man hadtumor of the trachea which was difficult to be differentiated from laryngeal and thyroid cancer. Biopsy of the tumoru was easily performed with an direct laryng-fiberscope. The tumor was squamous cell carcinoma located at the level of 2nd tra-cheal cartilage, and had an irregular and broad base, and occupied almost 50% of the tracheal lumen. The tumor was completely removed with combined total laryngectomy, bilateral neck dis-sections. The patient died 4 months after the operation due to secondary pulmonary adenoca-rcinoma. Squamous cell carcinoma of the trachea was very rare in Japan, but the knowlege for this disease and carefull examination of the trachea are neccesary.
