A clinical evaluation was carried out of 17 cases of free jejunal transfer for repair of defects after resection of malignant tumors of the hypopharynx or cervical esophagus. There were no problems invasculomicroanastomoses, and all operations were successful. Two marginal recurrences and 2 cervical lymphnode recurrences were found after complete resection. In the extensive lesions to the thoracic esophagus or double cancers, total resection of the esophagus and reconstruction with gastric pull-up was required. On the other hand, in the localized lesions within the hypopharynx or cervical esopha-gus, it was recommended that free jejunal transfer is a first choice for reconstruction.
