放射線治療が著効した気管原発扁平上皮癌の1例

A Case of Primary Tracheal Carcinoma Treated and Completely Responded with Radiotherapy

　はじめに

　気管原発の腫瘍は全剖検例中0.024％，悪性腫瘍剖検例中0.046％^1）と比較的稀な疾患である。本腫瘍に関する報告^2〜11）胸部外科医や呼吸器科医によるものが多くを占めるが，頸部気管に限局するものでは耳鼻咽喉科医が遭遇する機会もあるのではないかと思われる。今回われわれは，頸部気管粘膜より発生した扁平上皮癌における放射線治療著効例を経験したので報告する。

A 78-year-old woman with a complaint of stridor was found a tumor in the cervical trachea, andpathological diagnosis was moderately differentiat-ed squamous cell carcinoma. She rejected surgical therapy, and she was treated with external beam irradiation 40Gy (with CBDCA 75mg/m²/week, 3 times) and brachy therapy (¹⁹²Ir thin wire) 20Gy. Complete response (CR) was obtained and there was no apparent recurrence was found over 1 year.

A combination of external irradiation and brachy therapy should be considered for inoperable patients with localized tracheal squamous cell carcinoma.
