縦隔洞へ進展した深頸部膿瘍の1例

Deep Neck Abscess Sprcading into the Mediastinum:Report of a Case

　はじめに

　深頸部膿瘍は，抗生物質の進歩に加えて，CT，MRI等の画像診断が普及して病巣の拡がりを的確に診断し迅速に対処することが可能となったため，死に至ることは比較的稀となった。しかし依然として楽観できない疾患のひとつであることに変わりはない。なかでも，縦隔洞へ進展した場合はしばしば致命的となるため，縦隔洞への進展の有無を的確に診断し迅速に対処することが要求される。

　今回著者らは，縦隔洞へ進展し膿瘍を形成したため開胸ドレナージを実施し救命し得た深頸部膿瘍の症例を経験したので，その臨床経過を報告するとともに，縦隔洞膿瘍に対する開胸ドレナージの有用性に関して文献的考察を加えて報告する。

A 21-year-old male developed deep neck ab-scess. In spite of our intensive care, antibiotics and drainage, the abscess spread into the media-stinum. We were forced to drain it by thoraco-tomy. After that treatment, the patient recovered satisfactorily. It is our conclusion that thoraco-tomy for mediastinal abscess resulted in the higher survival rate than those of other treat-ments.
