　はじめに

　側頭骨高分解能CTは骨および軟部組織の描出に優れ，近年，鼓室形成術の術前検査法の一つとしてほぼルチーンに行われている。しかし，術前CT所見と手術所見が一致しない場合もあり，高分解能CTの限界に関しても検討する必要がある。我々は，鼓室形成術初回手術を施行した真珠腫性中耳炎症例につき，この点の分析を行い，高分解能CTの診断限界および画像描出上の特徴に関して若干の知見を得たので報告する。なお今回，上鼓室型真珠腫，後上部型真珠腫等の病型に関する検討は行わなかった。

Preoperative CT findings of the temporal bone were compared with operative findings with respect to bone destruction in 28 paients with cholesteatoma. In both the scutum and lateral semicircular canal, CT findings tended to be comparable with operative findings. In pre-operative CT, the false findings of bone destruction of ossicles (head of malleus and body of incus) and aditus ad antrum were commonly found, but the precise findings of bone destruction of tegmen were not always detected. In both the stapes and the tympanic portion of the facial nerve canal it is difficult to evaluate the state of bone destruction correctly only with CT.
