 Oral streptococci are rarely isolated in middle ear infections. We reported an unusual case of middle ear cholesteatoma infected with oral streptococci. A 31-year-old man presented with a 4-month history of right-sided hearing loss with otorrhea and dizziness. CT scan and intraoperative findings revealed destruction of the auditory ossicles, bone defects in the middle and posterior cranial fossae, and a fistula of the lateral semicircular canal. Streptococcus sanguinis and Streptococcus constellatus were isolated in the mastoid antrum. As shown in the present case, we should be aware that oral streptococci could cause severe middle ear infections.
