　はじめに

　外リンパ瘻は，内耳窓より鼓室内に外リンパが漏出する病態で，昭和58年に診断基準が呈示されその認識が高まっている。

　内耳窓破裂の原因として，先天性奇形に伴うものや手術，腫瘍，外陽などの後天性のもの，さらに原因不明（特発性）のものがあげられる。

　外傷性外リンパ瘻は，1968年Feeら^1）が初めて報告して以来，本邦でも数多くの報告例が認められる^2〜8）。しかし，経外耳道直達外力による外リンパ瘻症例の報告は比較的稀である。今回われわれは耳カキなどの直達外力により外リンパ瘻をきたした6例を経験したので，その症状，所見，術後経過などにつき若干の文献的考察を加えて報告する。

In six cases of perilymph fistula due to direct trauma, the causes were ear pick in five patients and a wire stuck in one patient.

Three cases showed trauma at the oval window, two cases at the round window and one case at both of them. All but one cases had auditory symptoms, and four cases had vestibular sympto-ms. In case of rupture of the oval window, there were strong vestibular symptoms.

The window membrane was repaired surgically in all cases, and immediate operation after trauma resulted in improvement of hearing and vestibular symptoms.
