内リンパ水腫を伴ったSystemic Lupus Erythematosus （SLE）の1症例

Endolymphatic Hydrops with Systemic Lupus Erythematosus (SLE):Report of a Case

はじめに

　メニエール病は，内リンパ水腫を代表する疾患で，めまいと難聴，耳鳴などの蝸牛症状を反復する。内リンパ水腫の原因として従来より，代謝障害説，アレルギー説，内分泌障害説，感染説など種々の原因が考えられている。さらに，近年自己免疫説が要因の一つとして加わってきており，McCabe^1）が自己免疫性感音難聴と言う概念を述べて以来，短期間に進行する感音難聴や変動する感音難聴の原因の一つとして自己免疫疾患が考えられるようになってきた。また，膠原病に合併した，感音性難聴の報告が多数なされており，メニエール病の中にもアレルギーや膠原病がその発症に関与しているとの報告が認められるようになってきている^2）。

　今回，突発的に起こった難聴，めまいを主訴に耳鼻咽喉科を受診し，当初突発性難聴と診断し副腎皮質ホルモン治療により回復したが，その後，難聴，めまいを繰り返し諸検査の結果，内リンパ水腫（メニエール病）と診断した，Systemic lupus erythematosus （SLE）に内リンパ水腫を併発した症例を経験したので報告する。

A 34-year-old woman was diagnosed at the age of 29 years as having SLE. She complained of bilateral ears fullness and a hearing loss in her right ear without dizziness. One week later, she developed profound dizziness, nausea and vomiting. At first, steroids was administered and reduced gradually under the diagnosis of sudden deafness, and consequently her hearing was restored. Several days later, however, the hearing deteriorated again and there occurred attacks of dizziness.

Since the glycerol test was positive and attacks of dizziness and hearing loss occurred repeatedly, endolymphatic sac operation was performd. After operation there was no attack, and improvement in the hearing and relief from dizziness were noted.
