頸部リンパ節転移による頸動脈過敏症候群の3例

Three Cases of Carotid Sinus Hypersensitivity Syndrome:After Cervical Lymplmode Metastasis

　はじめに

　頸動脈洞過敏症候群は，腫瘍が頸動脈に浸潤や圧排されることなどにより，頸動脈洞が刺激され，過敏状態となり，それによる徐脈・低血圧を主徴とする種々の症状をきたす疾患である。頭頸部腫瘍の頸部リンパ節転移を有する患者にしばしば認める。今回われわれは3例の頸動脈洞過敏症候群を経験したので報告する。

Three cases of carotid sinus hypersensitivity syndrome were reported and discussion of the anatomy and etiology of this syndrome was given. Case 1 was a 66-year-old male presented with T₀N₂M₀ cancer of unknown origin. Case 2 was a 49-year-old male presented with T₄N₃M₀ cancer of the hypopharynx and case 3 was a 59-year-old male presented with T₂N₀M₀ cancer of the tongue.

The conclusions are as follows: 1) This syndrome was caused by tumor involv-ing the carotid sinus.

2) The attack of this syndrome was immedia-tely improved by removal of parasympathetic state.

3) This syndrome disappeared by destruction of pressure receptor of carotid sinus.

4) The attack of this syndrome may result in serious cerebral infarction, therefore, prompt treat-ment and careful observation are required.
