頸部手術に合併した気胸，縦隔気腫

Pneumomediastinum and Pneumothorax following Neck Surgery

　はじめに

　気胸と縦隔気腫は，古くから頸部手術，特に気管切開術の合併症の1つとして知られている^1）。これらは頻発するものではないが，発症は突然でかつときに激烈であり，診断が遅れると致命的になる^2）こともある。この意味において頸部手術を扱うわれわれ耳鼻咽喉科医にとって本合併症について十分な理解をもつことは極めて重要であると考える。最近，頸部手術に気胸，縦隔気腫を合併した3例を経験したのでこれらを報告し，文献的考察を加えた。

A case of pneumothorax combined with pneu-momediastinum, a case of pneumothorax and a case of pneumomediastinum following neck sur-gery are presented. The cause of these complica-tions was thought to be due to an air-sucking mechanism at the root of the neck. These com-plications could be avoided by the thorough knowledge of anatomy, endotracheal intubation and the cares not to increase the airway pressure.
