頸部転移リンパ節治療後に生じた頸動脈洞反射亢進症の1例

A Case of Carotid Sinus Syndrome secondary to Hypopharyngeal Carcinoma and Left Neck Nodes Metastasis

I．はじめに

　頸動脈洞反射亢進症とはわずかの刺激で頸動脈洞反射による血圧低下，徐脈，あるいは失神などの症状が起こされるものをいう。

　本症は頭頸部腫瘍に起因したり，あるいはその治療に伴って発症することがあり，頭頸部外科医として念頭に置くべきものであるが，この領域での報告は本邦では7例をみるのみである。

A 72-year-old man had been treated by irra-diation to the neck and left radical neck dissec-tion.

One year after the surgery, he fell into syncope when the left carotid bifurcation was palpated. The syncope was reproducable and ECG revealedcardiac arrest during the palpation.

He was diagnosed as hypersensitivity of the carotid body which was related to the arterio-sclerosis and additional insults to the carotid body by irradiation and neck dissection.
