甲状腺の偶発乳頭状腺癌—頸部リンパ節腫大の取り扱い方

Occult Cancer of The Thyroid

緒言

　最近UICCのTNM分類において甲状腺癌の分類に変更があり，腺内の直径1cm以下の癌をT1とするなどその大きさを重要視する方向にある。これは甲状腺に早期癌の概念がない現在，できるだけ癌そのものが小さいうちに早期発見・早期治療をせんとする傾向にあることに他ならない。

　直径1cm以下のいわゆるT1例は以前より小型癌とか微小癌と呼ばれ，触診のみでは発見することさえ難しく，しばしば頸部の転移リンパ節を認めた後に，その原発巣検索の際に初めて発見されることが多い。頸部リンパ節の転移巣が先に発見された甲状腺癌はoccult cancerと呼ばれることが多いが^1），リンパ節生検材料から予期せぬ甲状腺癌が発見された場合はこれを特に偶発癌（inci—dental cancer）と呼ぶことがある^1）。

Thyroid occult cancer is defined as a cancer that is not clear as the primary lesion. This cancer is always small and difficult in palpation, therefore we often miss in palpation.

We report a thyroid incidental cancer that is found during the course of examination for sus-pected malignant lymphoma. A female aged 48 showed high antibody titer of ATLA adult (T-cell leukemia antigen) and atypical leukocytes with cervical lymphadenopthy.

Although no further evidence of ATLI, was found, a histopathologic study showed papillary adenocarcinoma in a cervical lymph node.

A total thyroidectomy with bilateral neck dis-section was performed, and papillary adenocar-cinoma was found in the thyroid.
