　緒言

　頭頸部悪性腫瘍の治療には抗癌剤による化学療法や免疫療法，放射線療法が普及し，その治療成績は向上を続けているが，ある程度進行した上皮性悪性腫瘍の場合には依然として根治的手術療法が必要とされ，治療成績のさらなる向上のためには腫瘍を含めた広範切除が原則とされる。しかし頭頸部という解剖学的特殊性から，美容上の問題点，さらに機能上の問題点を考慮せねばならず，一概に広範切除が唯一絶対というわけにはいかない。

　舌・口腔底を中心とした口腔内悪性腫瘍の場合，美容上の問題のみならず構音・咀嚼・嚥下といった生活の中心となる機能上の問題点を抱えており，手術的治療を施行した際の欠損部の再建が必要である。

In 33 cases of oral cancer, postoperative defects of the oral cavity were reconstructed immediately after radical surgery using various materials during the year of 1978 to 1989.

The age of the patients ranged from 27 to 74 years old, and there were 23 males and 10 fema-les. Primary sites of cancer originated in the tongue in 18 cases, floor of mouth in 6, gingival mucosa in 5, and others in 4, respectively. There were 16 cases in stage Ⅲ and 10 in stage IV. Seventeen cases were T2 and 8 were T3. A pectoral major musculocutanecus island flap was used in 22 cases, a delto-pectoral flap in 3. The various degrees of complications after reconstruc-tion occurred in 16 cases, and in 11 of 22 cases reconstructed using a pectoral major flap.
