甲状腺分化癌の131I治療法に関する臨床的検討

Clinical Evaluation of the Radioiodine Treatment for Differentiated Thyroid Carcinoma

緒言

　甲状腺分化癌の増殖は甲状腺刺激ホルモン（TSH）に依存性があると考えられ，病巣はヨードを摂取する性格を有し，場合により甲状腺ホルモン産生さえも行うとされている。一方，甲状腺分化癌はその増殖は遅いものの，治療は外科的治療が主体となり，化学療法や放射線外照射療法は適応外とさえ考えられている。しかし外科的治療のみでは根治性が得られない症例があることも確かである。そこでこのような根治不能もしくは再発した甲状腺分化癌に対し，その生物学的特性を生かして，1946年以後¹³¹I治療が諸施設において行われるようになった。¹³¹I治療は他臓器癌には見られない甲状腺癌に特異な治療法であり，かつ治療効果が高く評価されるものである。

　われわれの施設でも，放射線科において¹³¹I治療が行われており，1980年からは耳鼻咽喉科において外科的治療がなされた根治不能例，または再発例に対し同治療が選択され，症例数も11例を数えるに至った。今回この11例について検討を行ったので，若干の文献的考察を加え報告する。

In the last 9 years, 11 patients of well differe-ntiated thyroid carcinoma were treated with 100 mCi of ¹³¹I following thyroidectomy. Total thyr-oidectomy plays an important role to promote the uptake of the radioiodine into residual lesions and metastatic lesions. Five patients with recurrent thyroid cancers who had uptake of radioiodine showed complete response to this therapy. Our radioiodine therapy consists of a longer period of hypothyroidism. We do not performed a scin-tigraph with ¹³¹I before the therapy. Ten of 11 patients were satisfactory for this modality.
