鼻唇溝皮弁により即時再建を行った口腔内疣状癌の1例

Immediate repair of mucosal defect in the oral cavity with nasolabial flap: a case of intraoral verrucous carcinoma

はじめに

　口腔内悪性腫瘍の手術症例では，切除部位や再建術式によって術後に経口摂取や会話などが一定期間制限される。入院，手術，経鼻経管あるいは静脈栄養，会話制限などは高齢者にとって精神的，肉体的に大きな負担となるため，認知症の発症や増悪を招くことがある。疾病の根治性を損なわず手術侵襲を小さくすること，機能性，審美性の損失を少なくすること，再建術を含め手術回数を減らし，治療期間の短い術式を選択することは患者負担の軽減にかかわる重要な要素である。今回，広範な口腔内疣状癌の切除術後に残された頬部から下口唇裏面の欠損創を同側の鼻唇溝皮弁を用いて再建し，早期に退院させることができた症例を経験した。耳鼻咽喉科医にとり利用価値の高い鼻唇溝皮弁の一使用例として報告した。

　A 75-year-old woman presented with intraoral verrucous carcinoma. The tumor located in the entire lower gingiva, a part of left side of the oral floor, leading to the lower lip mucosal surface from the left cheek. After tumor resection wide defect remained on the left cheek and lower lip. The inferiorly based left side nasolabial flap was made and moved into the oral cavity through cheek tunnel. Nasolabial flap is good option for intraoral reconstruction especially in elderly patients with carcinomas. The flap requires one stage and less invasive surgery and allows early discharge from the hospital.
