頸部気管に原発したAdenoid cystic carcinomaの2例

Primary Adenoid Cystic Carcinoma of the Trachea:Report of Two Cases

　はじめに

　原発性気管腫瘍の中で腺様嚢胞癌は過半数を占めるが，気管腫瘍そのものの発生頻度が極めて低いため，わが国ではもっぱら症例報告として報告されており，その外科的治療に関して十分な経験を持っている施設は少ない。従って，このような症例に遭遇した際，どの様に診断，治療をすすめていくかについてはなお諸種の問題点が残されている。

　われわれは最近頸部気管に原発した腺様嚢胞癌2例を経験した。そこで，これら2例の治療経過を報告し，本症の治療について若干の文献的考察を加えた。

Two patients with adenoid cystic carcinoma of the cervical trachea were treated by segmental tracheal resection with primary reconstruction and postoperative radiation therapy. Although residual microscopic lesion was found in one patient, both patients were alive and clinically free of tumor at one and three years respectively after treatment. It is suggested that resection and primary reconstruction of the trachea for adenoid cystic carcinoma, usually with adjunctive irradition, provide long term palliation or cure.
