頭頸部adenoid cystic carcinomaの臨床像

CLINICAL FEATURES OF ADENOID CYSTIC CARCINOMA OF THE HEAD AND NECK

　頭頸部領域に発生する上皮性悪性腫瘍の多くは扁平上皮癌であり，腺癌の発生をみることは比較的少ない。しかし，当該領域においても，唾液腺・皮膚小器官などより腺癌の発生をみることがあり，多彩な組織像を呈するが，中でもadenoidcystic carcinomaはその臨床経過からも特異な位置を占めるものである。

　adenoid cystic carcinomaははじめBillroth^1）によりcylindromaという名称で報告され，1953年，Footeら^2）によりはじめて"adenoid cystic carcinoma"と記載されるようになつた。その間，当腫瘍は色々の名称で記載されている。cylindroma，basiloma，adenocystic basiloid carcinoma，adcnocpitheliomaなどの名称で呼ばれた腫瘍は，現在のadenoid cystic carcinomaと同一のものである。

　Ninteen cases out of our 34 malignant salivary gland tumors were adenoid cystic carcinoma. The site of origin in most of them appeared to be the palate, paranasal sinus and the parotid gland.

　Focal recurrences and distant metastasis were numerously met with.

　Radical operation is recommended as the first choice of treatment. Irradiation and chemotherapy appeared to be less effective in these cases, however, there were a few cases which were radiosensitive. Radiotherapy for inoperable eases or postoperative irradiation is permissible. Secondary surgical removal of the recurrent tumors should be actively persued.
