　はじめに

　急性副鼻腔炎による眼窩内合併症は，抗生物質の開発，発達により著しく減少しているが，現在もなお副鼻腔炎による眼窩内合併症が報告されている^1）。原因として，副鼻腔と眼窩は解剖学的に隣接するため副鼻腔の炎症が直接的に波及しやすいこと，副鼻腔の静脈還流も眼静脈を経由して海綿静脈洞に流入することなどが考えられる。

　今回我々は，抜歯に引き続き起こった急性副鼻腔炎の炎症が眼窩内に波及し，眼窩蜂窩織炎となり，手術を要した症例を経験した。未だ明確にどのような状態，または病期で手術的治療に踏み切るべきか述べられたものは少なく，今回経験した症例を報告するとともに，病因，感染経路，さらに手術的治療の適応について考察した。

Orbital cellulitis can be possible after acute paranasal sinusitis. Surgical or conservative treat-ment must be decided by the stage of the disease. Surgical drainage should be required when visual impairment, orbital and subperiosteal abscess were found and it should be kept in mind that surgical treatment may be d manded in cases of proptosis and limitation of extraocular movement. Drainage should be performed even if there is no evident signs because of severe conjunctivitis and edemtous eye-lids. We discuss indications for surgical treat-ment to orbital cellulitis and the usefulness of surgical therapy.
