はじめに

　誘発電位は中枢神経系の機能診断をする上で重要な検査法となっている。とりわけ聴性脳幹反応（Auditory Brainstem Response；ABR）は聴神経腫瘍を始めとする小脳橋角部腫瘍の手術中に，脳幹部の機能状態を知るためのモニターとしても定着しつつある^1〜4）。

　しかし，脳幹そのものの病変に対しては，脳幹部の機能的重要性から外科的治療が行われることは少なく，保存的治療が好まれる傾向にある^5,6）。そのため脳幹内部への直達手術の際の術中ABRモニタリングの報告例は少なく，ましてや脳幹部海綿状血管腫では直達手術例の報告自体がわずか数例^5,6）であるため，その術前・術中・術後にわたるABRモニタリングとなると稀有である。

A 34-year-old male underwent a surgical remo-val of hemangioma in the pons. Preoperative ABR revealed increases in interpeak latences (IPL) of left I-III and bilateral III-V waves, and abno-rmally separated bilateral IV/V waves. Infra-operatively, ABR became unstable and waves of IV and V disappeared just after pontine incision. However, all waves of ABR could be detected at the end of operation. Postoperatively, prolongations of IPL decreased bilaterally, and separation of IV/V waves became improved in both ears. Intraoperative monitoring of ABR could state that the hearing acuity and brainstem function were preserved in spite of pontine surgery.
