はじめに

　再発性多発性軟骨膜炎（relapsing polychondritis：RP）は，系統的に全身の軟骨組織を侵す自己免疫疾患である。診断は臨床症状と病理生検に基づいて行われるが，症状は多彩で出現時期も一定しないため，確定診断に至るまでさまざまな疾患が鑑別に挙げられる。今回われわれは，慢性中耳炎急性増悪と鑑別を要し，経過とともに症状が出現して確定診断に至った症例を経験したため，若干の文献的考察を加えて報告する。

　Relapsing polychondritis is a rare autoimmune disease. Only a few patients with this disease present with typical manifestations and symptoms generally develop with time. We report a 74-year-old man who presented with right temporal pain and pyrexia. Computed tomography showed a shadow in non-pneumatized mastoid antrum, which required differential diagnosis from acute exacerbation of chronic otitis media. Bilateral auricular swelling and ocular inflammation developed with time, and auricular biopsy led to a definitive diagnosis of relapsing polychondritis.
