癒着性中耳炎に対する内視鏡下subannular tube挿入例の検討

はじめに

　癒着性中耳炎は，鼓膜が陥凹し中耳腔の内側壁と癒着した病態である。中耳粘膜の線毛機能やガス交換能，耳管機能が高度に障害されており，手術を施行しても鼓膜が再癒着することも少なくない。種々の手術の工夫の報告もみられるが，他の中耳疾患に比べ手術成績が良好ではない。

　subannular tube（SAT）とは，菲薄化した，または鼓室内に接着・癒着した鼓膜を介することなく，外耳道皮下から鼓膜輪下を経由して挿入するチューブのことで，チューブ脱落や脱落後の鼓膜穿孔をきたす可能性が低く，長期留置が可能という報告もある。

　2016年より当院では，癒着性中耳炎をはじめとする難治性中耳換気障害に対し，SATを取り入れてきた。今回，当院においてSAT挿入を施行した症例について検討を行ったので報告する。

　The subannular tube（SAT）is a simple method in which T-tube is inserted subannularly via a tympanomeatal flap from the external auditory canal wall to the middle ear, leaving the tympanic membrane intact. We reviewed 8 cases of patients with adhesive otitis media, atelectatic ear, and cholesteatoma who underwent transendoscopic SAT insertion. Although in 4 patients the tube was not retained, 7 patients have been doing well without complications. The SAT method provides a safe and effective option for management of adhesive otitis media.
