傍咽頭間隙へ迷入した下顎智歯の1例

A case of foreign wisdom tooth extracted to parapharyngeal space

はじめに

　抜歯操作は日常歯科診療において頻繁に行われる手技の1つである。その合併症として，歯牙の口腔底や上顎洞への迷入がある^1-3）。迷入歯とは異所へ迷入した歯牙をさし，下顎の迷入歯は智歯に特に生じやすい。この理由は舌側歯肉が歯槽骨から容易に剝離されやすいこと，下顎舌側歯槽骨が菲薄で破折，穿孔しやすいためである。そして，大部分は前方，舌側に迷入する^4）。

　一方，後方である傍咽頭間隙の迷入歯はきわめて珍しく，その報告はない。今回われわれは傍咽頭間隙へ迷入した下顎智歯を経験したので報告する。

　A 31-year-old female was recommended the removal of a wisdom tooth because it would be cause of infection, and the tooth had been removed. However, it lodged into the parapharyngeal space accidentally. After 11 days, she had disturbance of opening mouth, and visited to our hospital. CT and orthopantomography showed the foreign wisdom tooth in left parapharyngeal space. It was removed surgically under general anesthesia.

　Foreign body tooth should be removed immediately, because it would be cause of fatal infection disease.
