外切開・下咽頭部分切除術に内視鏡手術を併用した下咽頭癌の1例

A case of partial hypopharyngectomy via external incision combined with endoscopic laryngopharyngeal surgery

はじめに

　下咽頭癌に対する喉頭温存手術として，外切開による下咽頭部分切除術や，endoscopic laryngopharyngeal surgery（ELPS）などの経口的切除が行われている。外切開・下咽頭部分切除術は，腫瘍深部の切除を行うことができるが，切除の際に咽頭外から咽頭腔に進入するため，詳細な切除ラインを設定することが困難な場合がある。一方でELPSなどの経口的内視鏡手術は，narrow band imaging（NBI）などの画像強調内視鏡や拡大内視鏡を用いて，腫瘍の粘膜面の詳細な切除ラインを決定することができるが，深部浸潤のある腫瘍の切除への適応は限られる。今回われわれは，下咽頭癌放射線治療後局所再発の救済手術として外切開による下咽頭部分切除術にELPSを併用した1例を経験したので報告する。

　Partial hypopharyngectomy via an external incision and transoral surgeries, such as endoscopic laryngopharyngeal surgery（ELPS）, are larynx-preserving surgeries for hypopharyngeal cancer. In partial hypopharyngectomy via an external incision, it is possible to excise the deep layer of the tumor, although it is not possible to determine a detailed resection line of the mucosal surface of the tumor.

　Here, we report a case of salvage partial hypopharyngectomy via an external incision combined with ELPS for hypopharyngeal carcinoma after radiotherapy.
