 Lingual osseous choristoma is a rare ectopic lesion of the tongue. To date, only 54 cases have been reported in Japan, and fewer than 100 in the English literature. Although most cases are managed under local anesthesia in outpatient settings, surgical excision can be difficult depending on the lesion's location, particularly if it triggers a gag reflex.

 We present a case of lingual osseous choristoma at the sulcus terminalis, resected via transoral approach. We also discuss the advantages of general anesthesia for lesions in anatomically challenging sites.
