佐藤式彎曲型喉頭鏡が有用であった下咽頭梨状陥凹瘻の1例

A case of pyriform sinus fistula teated under Sato's curved laryngoscope

はじめに

　近年，光学内視鏡の発展とともに下咽頭喉頭の観察方法も工夫がされるようになってきた^1）。佐藤式彎曲型喉頭鏡もその1つで，従来は観察困難であった下咽頭粘膜病変も観察良好となった。下咽頭梨状陥凹瘻は，第3または第4鰓囊から生じる先天性瘻孔で，深頸部膿瘍や急性化膿性甲状腺炎の原因となる^2,3）。そのため，日常臨床では小児や若年者の反復する頸部膿瘍の原因疾患として遭遇する。根治治療は瘻管の完全摘出あるいは閉鎖であり，瘻管の同定が不可欠である。一方，従来の直達喉頭鏡や内視鏡による観察方法では瘻孔同定が困難で，結果的に再発する症例も散在した。

　佐藤式彎曲型咽喉頭鏡は下咽頭の良好な視野を確保できるが，これを瘻孔同定に使用した報告は本邦で1例しかない^4）。今回われわれは，佐藤式彎曲型咽喉頭鏡を用いた観察により瘻孔の同定方法を追試し，瘻管の完全摘出が可能であった1例を経験したので報告する。

　The recent development of endoscopes has expanded our ways to observe the hypopharynx. The Sato's curved laryngoscope enabled us to observe the mucosal lesion of pyriform sinus, which was difficult to be observed using previous laryngoscopes.

　Pyriform sinus fistula is a congenital fistula, which causes deep neck infections. Surgical excision is considered to be the definitive therapy, and identifying the fistula opening is essential.

　We presented the case of a 22-year-old woman who was diagnosed with pyriform sinus fistula. We identified the fistula by performing an intraoperative endoscopic examination with the Curved laryngoscope, which ultimately led to a complete excision.
