はじめに

　外耳道がんは頭頸部悪性腫瘍全体の約1％と比較的稀な疾患で，臨床の場で遭遇する機会は多くはない^1）。また，発症早期は症状に乏しく，出現する症状も耳漏や耳痛といった非特異的なものが主であるため，外耳炎や中耳炎として加療される場合も少なくない。さらに，周囲の重要臓器へ容易に浸潤する部位にあるため，進行した段階で悪性疾患と診断されることが多く，根治治療には難渋する。近年外耳道がんに対しては，外科的切除のみならず，サイバーナイフによる放射線療法や，超選択的動注ならびに放射線化学療法が有効との報告も認められる^2-9）。

　今回，当科で経験した外耳道がん症例11例について，臨床像・治療法・予後についてretrospectiveに解析し，当該疾患の治療について文献的考察を加え報告する。

　We reported treatment result of 11 patients with squamous cell carcinoma（SCC） of the external in our hospital. Most patients were elderly and advanced cases. As an initial treatment, CyberKnife, conventional RT, RADPLAT or TPF combination CRT was performed. The overall 3-year survival rate of T4 stage was 83.3％. All patients treated with RADPLAT or TPF combination CRT showed complete response. There were no severe adverse effects secondary to these treatments. These results suggested RADPLAT and TPF combination CRT are safe and effective as the function preserving treatment for patients with SCC of the external auditory canal.
