はじめに

　免疫チェックポイント阻害薬（immune checkpoint inhibitor：ICI）は再発・転移頭頸部がん治療におけるキードラッグであり，一次治療，二次治療どちらにも使用可能である。一方でICIは特有の免疫関連有害事象（immune related adverse event：irAE）を引き起こすことがあり，皮膚障害は頻度が30〜40％^1）と高いが，多くは非特異的な発疹や瘙痒，白斑であり^2），類天疱瘡は稀とされている。今回われわれはICI投与後に類天疱瘡を生じた1例を経験したので，ここに報告する。

　We report a case of pemphigoid that occurred as an immune-related adverse event（irAE） after administration of immune checkpoint inhibitor（ICI）. The patient was a 63-year-old man. He had been receiving treatment with nivolumab for recurrent olfactory neuroblastoma and developed pemphigoid two years later. The skin symptoms improved with steroid administration, but refractory pemphigoid recurred when nivolumab was restarted. Although the incidence of pemphigoid caused by irAE is rare, there is a risk of discontinuing ICI administration or a poor prognosis, therefore, treatment should be initiated promptly in cooperation with a dermatologist.
