はじめに

　側頭骨悪性腫瘍は比較的稀な疾患であり，発生頻度は100万人に1人であると報告されているが外耳道原発の癌はさらに少なく^1）日常診療で遭遇することは稀である。初期の外耳道癌は難治性外耳道炎などの炎症性疾患と鑑別困難で，早期診断が難しいことが多く^2），統一された病期分類や手術方法は定まっていない。また，われわれの渉猟しえた範囲において本邦で外耳道癌根治切除後の聴力予後を検討した報告はない。今回われわれは，比較的早期の外耳道癌に対して中耳温存手術を施行した10症例を後向きに検討した。

　We presented 10 cases of carcinoma of external auditory canal（EAC）treated by the middle ear preservation operation. Partial resection of the EAC was done in 4 cases for T1 case and en bloc resection of the EAC together with tympanoplasty in 6 cases for T1 and T2 case. Some of the patients who underwent en bloc resection of the EAC together with tympanoplasty preserved or improved their hearing after the operation compared with operative method of obliteration of the EAC. They had a good prognosis expect for a patient discontinued to visit the hospital. We considered that en bloc resection of the EAC together with tympanoplasty for T1 and T2 cases improves a high QOL of the patients.
