頭蓋骨膜弁を用いた大きな頭蓋底再建—鼻中隔軟骨肉腫症例

Large Skull Base Reconstruction with Pericranial Flap: A Case of Nasal Septal Chondrosarcoma

はじめに

　近年，頭蓋底に生じた，あるいは浸潤した悪性腫瘍に対し積極的に手術治療を行った報告を散見するようになった^1）。頭蓋底を切除するのを決断するのは，未だに容易なことではない。腫瘍はとれても，再建が問題になる。鼻腔側の粘膜が温存される場合は脳外科的アプローチにより遊離骨移植で済まされるが，全層にわたって欠損する場合は多角的アプローチが必要となる。つまり，切除，再建には経鼻的あるいは開頭によるアプローチをしなければならない^2〜4）。それには脳外科，耳鼻科，形成外科の集学的手術治療が必要となる。二方向からの手術をすることで切除だけでなく，再建も確実になされる。脳の嵌頓や感染などを防止するうえで非常に重要なことである。頭蓋底再建には脳圧および重力に耐えうる強度と確実な鼻咽腔側の上皮による被覆再建が必要である。

　我々は比較的悪性度が低い鼻中隔由来と考えられた軟骨肉腫症例で，広く頭蓋底を切除し，これを再建して良好な結果を得られたので，以下に報告する。

The reports of the skull base reconstruction surgeries have been recently increasing accom-panied with expanded excisions of head and neck malignancies. However, the technical security has not been completely established yet, and the indica-tions of operation has been discussed for each case.

We experienced one case with nasal septal chon-drosarcoma. The mass of this case deeply ex-panded into the intracranial space and the prognosis was diagnosed unfavorable without radical opera-tion because this tumor is low malignancy but resistant to radiotherapy and chemotherapy. Skull base was excised with full thickness bidirectionally from the nasopharynx and intracranially. The defect of the skull base was 4×5cm in large and was reconstructed with a pericranial flap, free skin graft beneath it and an outer table of parietal calvarium. At 6 months after the operation the reconstructed skull base steady and the general condition was favorable.
