頭蓋底硬膜下膿瘍をきたした鼻中隔膿瘍の1症例

Subdural Empyema of Skull Base Secondary to Nasal Septal Abscess

　はじめに

　抗生物質の発達した現在，鼻性頭蓋内合併症はまれな疾患となりつつあり，その報告も数少なくなっている。また，鼻中隔膿瘍も同様に近年ではまれな疾患といえる。しかし，現在でも治療開始時期の遅れや，糖尿病などの全身疾患の合併は，不測の事態を引き起こすことがある。最近われわれは，鼻中隔膿瘍と副鼻腔炎を合併し，さらに化膿性髄膜炎を併発して死に至り，剖検により頭蓋底硬膜下に膿瘍が証明された1症例を経験したので，若干の考察を加えて報告する。

A 65-year-old male with diabetes mellitus de-veloped a sudden nasal obstruction and impair-ment of visual activity. The patient was diag-nosed as having rhinogenous optic neuritis. Despite local drainage and administration of antibiotics, he died of meningitis. Subdural empyema was observed on the base of skull in autopsy, which could not be found on CT scan while the patient was alive.
