長掌筋腱付遊離皮弁による軟口蓋動的再建例の鼻咽腔閉鎖機能

Velopharyngeal Function after Reconstruction of the Soft Palate by a Free Flap with Long Palmar Muscle Tendon

　はじめに

　軟口蓋は構音，嚥下，鼻呼吸に重要な役割を果たしている。このため軟口蓋の全欠損による機能障害は極めて深刻で，軟口蓋を全切除した後の機能の修復は臨床上の大きな課題であり，今までに種々の再建手術が試みられてきた^1〜5）。しかし，鼻咽腔の開放と閉鎖という相反する機能を同時に満足させるのは，単に形態的に軟口蓋を再建する従来の方法では，非常に困難であった。

　今回われわれは，軟口蓋の全切除例に長掌筋腱付遊離皮弁または遊離筋皮弁を用いて一期的再建を行い^6），術後の鼻咽腔閉鎖機能について検討したところ，興味ある結果が得られたので報告する。

The velar reconstruction was performed using a free forearm flap or free myocutancous flap of the abdominal rectal muscle, attached with the long palmar muscle tendon, immediately after total removal of the soft palate due to malignant tumors in three cases. Satisfactory velopharyngeal function was achieved postoperatively by elevation of the reconstructed soft palate, movement of the lateral pharyngeal wall and the ridge at the posterior pharyngeal wall. Our results revealed that the long palmar muscle tendon was useful material as a framework in the functional recon-struction of the velopharyngeal mechanism.
