Ⅰ．はじめに

　輪状咽頭筋は食道入口部に存在し，安静時には一定の筋緊張を保つように持続収縮することで食道入口部を閉鎖させているが，嚥下運動時には弛緩して食道入口部を開大させる。輪状咽頭筋が弛緩不全となる病態はよく知られており日常的に経験するが，弛緩したままの状態が観察されることは珍しい。今回われわれは食道入口部が持続的に開大していた3症例を経験した。いずれの症例も胃・食道から咽頭への逆流が強く疑われ，嚥下性肺炎の原因となっていたと考えられる。症例提示に若干の文献的考察を加えて報告する。

　The esophageal entrance is closed by the persistent contraction of the cricopharyngeal muscle at rest. On swallowing the esophageal entrance opens by the relaxation of this muscle. Cricopharyngeal dysfunction is common pathology which is sometimes seen in the patients with dysphagia. However, we rarely find the continuous relaxation of the cricopharyngeal muscle and the continuous opening of the esophageal entrance. We presented three cases with continuous opening of the esophageal entrance. Gastroesophageal reflux inducing aspiration pneumonia was suspected in each case. The mechanism of continuous relaxation of the cricopharyngeal muscle and the prevention of aspiration pneumonia were discussed.
