ハーモニック®を用いた頭頸部手術

New technique with Harmonic® in head and neck operation

Ⅰ　はじめに

　ハーモニック^®〔Ethicon Endo-Surgery, USA，（以下，ハーモニックと略す）〕は内視鏡下外科手術で広く用いられている超音波凝固切開装置である。超音波エネルギーを利用した手術機器で，凝固切開能力に優れ，電気メスに比べて組織の熱損傷が少なく止血効果に優れる利点をもつ^1）。近年，ブレードの改良とともに頭頸部手術への応用も可能となり，その有用性についての報告も散見されるようになってきた^2～5）。当施設では2008年よりハーモニックを導入して頭頸部手術（頸部郭清，甲状腺，耳下腺，顎下腺など）を行っている。今回われわれはハーモニックの有用性と安全性を明らかにするために，ハーモニック導入前後の手術関連因子に対する影響について従来法と比較し検討を行ったので報告する。

　Harmonic is an ultrasonic dissector,which is widely used for the endoscopic surgery. We have applied Harmonic for head and neck operation since 2008. To elucidate the utility and safety of Harmonic,we investigated the effect of Harmonic for the operation related factors. The operation time and the total blood loss were significantly decreased by the use of harmonic. We have not encountered post-operative hemorrhage. Harmonic proved to be very useful and safe for head and neck operation. And it will play an important role for the head and neck surgery in the near future.
