Ⅰ．はじめに

　喉頭癌の99％は扁平上皮癌であるが^1），喉頭は肺外小細胞癌の好発臓器でもある。ただしこの頻度は喉頭癌の0.3％に満たず^2），過去30年で国内外において喉頭小細胞癌の報告は検索した限り160例である^1）。扁平上皮癌を含めた非小細胞癌と小細胞癌が混在する混合型小細胞癌はさらに稀で，これまでの報告は16例である^2）（表1）。症例数がきわめて少ないため，いまだ標準的な治療法も確立されていない。腫瘍は一般に，高度に進行性で早期に遠隔転移する特徴を有し予後不良であり，多くが2年以内に死亡するとされている^3）。

　今回われわれは，左声門上部に発生した混合型小細胞癌を経験したので，文献的考察を加えて報告する。

　Combined small cell carcinoma（CSCC）consists of small cell carcinoma and non-small cell carcinomas including squamous cell carcinoma. CSCC derived from the larynx is very rare,and only 16 cases have been described to date. We herein reported a case of supraglottic CSCC（T3N2bM0）,which postoperatively diagnosed as CSCC with squamous cell carcinoma. Because CSCC is generally aggressive and invasive with poor prognosis,multimodality therapy including radiation and adjuvant chemotherapy（cisplatin＋etoposid）was chosen. Although our case relapsed in the neck,a continuous disease-free condition was acquired after salvage surgery and additional adjuvant chemotherapy（amrubicin）.
