Ⅰ．はじめに

　川崎病は4歳以下の乳幼児に好発する原因不明の血管炎であり，血管炎に起因すると思われるさまざまな臨床症状および所見を呈することが知られている^1）。最も重要な合併症として知られる冠動脈瘤の形成は予後にかかわるため，迅速な診断および治療を必要とする。

　今回，われわれは疼痛を伴った左頸部腫脹を主訴とした咽後蜂窩織炎を合併した川崎病の1例を経験した。咽後膿瘍との鑑別が画像上困難であり，診断・治療に苦慮した症例であったため，若干の文献的考察を加えて報告する。

　A 13-year-old patient presented with a cervical swelling, pain and high fever. Cervical lymphadenitis was suspected and the patient was treated with antibiotics without relief of symptoms.

　CT of the neck showed several lymphadenopathies with a low density area in the retropharyngeal space. Retropharyngeal abscess was suspected and a needle aspiration was performed without drainage of pus. On the 8^th day, the patient developed multiple erythematous skin eruptions in his trunk and finally he was diagnosed as having Kawasaki disease.
