第Ⅷ脳神経症状を初発とした原発性肺癌内耳道内転移の1症例

A case of primary lung cancer with 8th cranial neuropathy as the first sign of onset

Ⅰ．はじめに

　小脳橋角部や側頭骨領域を占拠する転移性腫瘍病変に遭遇する機会は臨床上少ない。今回われわれは，右難聴，回転性めまいで発症し，その後，遅発性に右顔面神経麻痺が出現した原発性肺癌の内耳道内転移症例を経験したので，若干の文献的考察を加えて報告する。

　A 58-year-old man suffered from right hearing loss and rotatory vertigo, however his condition was considered as sudden sensorineural hearing loss. One month later, right facial palsy was developed and he was admitted to our hospital.

　An abnormal shadow was detected in the left upper lung field and adenocarcinoma of the lung with multiple brain metastasis was diagnosed. Cerebrospinal fluid cytology revealed metastatic meningeal cartinomatosis. He underwent systemic chemotherapy consisting of combined carboplatin and paclitaxel, but he died about 5 months after the appearance of symptoms.

　We reviewed the detail of clinical features and discussed metastatic temporal bone malignancies.
