内耳道内と髄膜病変を呈した中枢神経ゴム腫の1例

A case of central nervous system syphilitic gummas involving the internal auditory canal and meninges

ゴム腫は梅毒に伴う非特異的な壊死性肉芽腫病変であり，皮膚・粘膜・骨のほか，あらゆる臓器に生じ得る^1）。通常ゴム腫は晩期梅毒において認められるが，human immunodeficiency virus（HIV）感染を合併した患者の場合，比較的感染早期でも発生する場合があることが知られている^2）。今回，我々はHIV感染患者にて多発髄膜病変と内耳道内病変の中枢神経ゴム腫を呈した神経梅毒の1例を経験したので報告する。

We report a rare case of HIV-infected man with CNS syphilitic gummas. MRI showed a well-enhancing lesion in the left auditory canal and the leptomeningeal enhancements in the frontal and temporal lobe surfaces. Although gumma is a rare manifestation of neurosyphilis, we have to keep in mind that gumma can occur even in the early stages of the syphilitic infection in HIV patients.
