視力障害を初発症状とした転移性肺癌の1例

A case of metastatic lung tumor with visual impairment as the initial symptom

（展示96）　34歳女性が右眼の視力低下と変視症で眼科を受診した。右眼後極部に黄白色の隆起性病変があり，転移性脈絡膜腫瘍が疑われた。CT検査で脳，肺，肝，子宮前方に腫瘍があり，Gaシンチグラムで肋骨，腸骨，腰椎に転移所見があった。卵巣と腸骨の腫瘍切除で，組織学的に肺原発の腺癌が強く疑われた。比較的若年者ながら，眼所見を契機として肺癌が発見された例である。

A 34-year-old female presented with visual impairment and metamorphopsia in her right eye. Funduscopy showed an elevated white mass in the posterior fundus. Computed tomography showed tumorous mass in the brain, lung, liver and ovarium. Gallium scintigraphy showed metastasis to the ribs, ilium and lumbar vertebra. Surgical removal of the tumor in the ovarium and ilium showed adenocarcinoma presumably originating from the lung. This case illustrates that ocular symptoms may lead to the detection of metastatic lung tumor even in relatively young person.
