I．はじめに

　現在，画像診断においてmagnetic resonance imaging（MRI）の果たす役割は大きく，めまい・平衡障害の診断においても頭部MRIは重要な役割の一端を担っている。一方，MRIの技術進歩に伴い，目的とする病変に合わせた撮像条件の選択も必要となってきており，単純MRIのみで器質的病変の変化をどこまで検出できているのかについては不明な部分もある。

　今回われわれは，当初，単純MRI画像所見上，異常を認めなかったものの，急速な平衡障害の悪化をきたし，造影MRIにて多発性脳転移が発見された肺小細胞癌の1症例を経験したので文献的考察を加えて報告する。

　MRI is one of the important diagnostic tools for patients with vertigo and/or disequilibrium. However, normal images of MRI cannot exclude all of the organic central lesions. We reported a 67-year-old man with small cell carcinoma of the lung presenting of acute disequilibrium. As brain MRI without enhancement showed no remarkable findings, he was treated at psychiatry department. His disequilibrium, however, continued, and neuro-otological evaluation strongly suggested central lesions. An enhanced MRI revealed multiple brain metastases. In this report, we emphasized that MRI without contrast enhancement does not exclude all space-occupying lesions.
