耳下腺唾石症に対して体外衝撃波結石破砕術が奏効した1症例

A case of parotid sialolithiasis successfully treated by extracorporeal shock wave lithotripsy

Ⅰ．はじめに

　耳下腺唾石の治療は，マッサージによる唾液分泌促進などの保存的治療や，場合によっては外切開による手術治療が行われている。しかし，良性疾患であるため，顔面神経損傷や唾液瘻，Frey症候群などの合併症の可能性を考慮すると，外切開による手術に踏み切るのが困難な場合もある。

　今回われわれは，若年女性の耳下腺唾石症に対し，比較的新しい手技である体外衝撃波結石破砕術（extracorporeal shock wave lithotripsy：ESWL）を施行し，良好な結果を得たので，文献的考察とともに報告する。

　Treatment of parotid siaolithiasis is often very embarrassing for clinicians. A 19-year-old woman with parotid sialolithiasis was treated successfully by extracorporeal shock wave lithotripsy（ESWL）. She had complained of continuous pain and a swelling of the left parotid gland, which was not improved by medication. ESWL was undertaken with no serious complications. Her symptoms were considerably decreased and the computed tomography imaging at 4 months after a trial of ESWL showed no stones. ESWL should be taken into consideration more often in patients with parotid sialolithiasis.
