I．はじめに

　アブミ骨手術は耳硬化症や中耳奇形に対して聴力改善目的に施行されるが，術後経過中に聴力低下をきたした症例も少数ながら認められる。そのような症例は，再手術の際に原因が発見される場合が多い。その1つとしてピストンなどのプロテーゼの偏位や脱落がある。

　今回われわれは，アブミ骨手術後ピストンの脱落を右2回，左1回繰り返した1症例を経験した。この症例について，われわれが行った脱落予防の対策を紹介する。

　A 28-year-old woman with bilateral stapes fixation underwent revision stapes surgery three times：twice for the right ear and once for the left,due to dislocated prosthesis. The erosion of the long process of the incus was found in all revision surgeries,and it seems due to the pressure by the fastened Teflon piston. Therefore,at the second revision surgery for the right ear,we chose a Teflon wire piston instead of a Teflon piston,fastened the wire loosely,and fixed the wire by a perichondrial graft to prevent its dislocation. The short-term hearing result（one year） was satisfactory.
