舌根部小唾液腺由来低悪性腺癌症例―正中下口唇下顎骨舌切断法によるアプローチ

A Case of Low-Grade Adenocarcinoma in the Minor Salivary Grands:Median Labiomandibular Glossotomy Approach for Resection of Base of Tongue Tumor

I．はじめに

　舌根部腫瘍は，手術のアプローチが難しく，その選択に難渋することがある。広い術野を得ることに越したことはないが，整容上の問題も考慮しなければならない。悪性度の高い腫瘍では，ある程度の侵襲もやむを得ないが，良性腫瘍や悪性度の低い腫瘍では，術野の確保とともに，低侵襲な手術が望まれる。舌根部へのアプローチ法としては口内法，下顎正中離断を用いる方法，舌骨上咽頭切開もしくは咽頭側切開を用いる方法などがあるが^1,2），それぞれに利点，欠点がありその選択には症例ごとの十分な検討が必要である。

　今回われわれは，舌根部低悪性腺癌に対して正中下口唇下顎骨舌切断法（median labiomandibular glossotomy）によるアプローチを用いて腫瘍を摘出した症例を経験したので報告する。

　A 72-year-old man complained of pharyngeal bleeding. He had a mass in the base of tongue with ulcer. Surgical resection was performed by median labiomandibular glossotomy with a small amount of bleeding and minimal surgical stress. This tumor was diagnosed as polymorphous low grade adenocarcinoma by histological examination.

　Median labiomandibular glossotomy allows a good exposure of the base of tongue. The advantages of this approach are minimal bleeding,excellent functional and cosmetic results.
