診断に苦慮した鼻腔結核の1例

A case report of nasal tuberculosis

Ⅰ．はじめに

　1920年ころには結核性疾患は数十万人の患者が存在していたが，1951年に結核予防法が改定されて以来，優れた抗結核剤の開発と結核対策の推進・公衆衛生の向上によって減少の一途を辿ってきた^1）。しかし1997年には新規結核登録患者数，罹患率が上昇に転じて1999年には旧厚生省（現厚生労働省）が結核緊急事態宣言を行った^2）。そして耳鼻咽喉科領域でも結核症の報告例は増加している^1）。

　今回われわれは若年女性の下鼻甲介に発生した結核症例を経験した。若干の文献的考察を加えて報告する。

□Abstract

　A 26-year-old female complained of nasal obstruction and sore throat. The clinical findings showed that the inferior turbinate and nasal septum were covered with exsudate. Multiple aphthae were recognized in the oropharynx and scattered papules were recognized in the face.

　The first biopsy from the nasal cavity showed inflammatory changes. The second biopsy revealed pathological diagnosis of tuberculosis. When tuberculosis is suspected, biopsy is recommended for the definite diagnosis.
