　52歳女性に，心筋梗塞後の頻拍に対して1日量200mgのアミオダロン投薬が開始され，14か月後に両眼の渦状角膜症が発見された。その後も投薬が続けられ，角膜浅層の渦状薄茶色の混濁は変動しながら持続した。角膜表面には異常がなかった。投薬開始から29か月後に死亡するまで視力障害はなかった。本症の角膜混濁がドライアイの涙液干渉像に類似することから，色素沈着は涙液を介する可能性がある。

A 52-year-old female started treatment with systemic amiodarone at the daily dosis of 200 mg for tachycardia following coronary infarction. Typical vortex-pattern keratopathy developed in both eyes after 14 months of treatment. The subepithelial corneal pigmentation persisted with minor fluctuations. No abnor-malities developed in the corneal surface. There was no visual disturbance until death of the patient after 29 months of treatment. The corneal opacity in this patient simulated the interference pattern of tear film in dry eye patients. There is a possibility that pigments in amiodarone keratopathy are derived via the tear fluid.
