A 74-year-old woman presented with inferior hemianopia in her right eye since 5 days before. Her corrected visual acuity was counting fingers right and 0.4 left. The right eye showed relative afferent pu-pillary defect. The optic disc appeared normal in both eyes. Magnetic resonance imaging (MRI) showed ret-robulbar neuritis in the right eye and empty sella. Optic neuritis developed in the left eye 3 weeks later. Vis-ual acuity in both eyes remained unchanged after corticosteroid pulse therapy. The empty sella may have con-tributed to visual loss in this patient. This case illustrates the usefulness of sagittal MRI in detecting empty sella as an underlying lesion in optic neuritis.
